DYT Productions Performance Studio - Payment Information

DYT requires 50% of the class fee at the time of registration. The remaining 50% will be due at the 5th class, the halfway mark of the term. By signing the registration form, you the parent/guardian, are agreeing to pay for the full term of class or classes that are listed above. If final payment is not received by the 5th class, a penalty of $25 dollars will be applied to the final bill for every week that the due amount is not paid. By week 7 of the term, if payment has not been received, the DYT student will not be allowed back into the class, and the penalty fee will continue to be added. In the event that a student wishes to not continue the class, NO REFUND will be issued. If a student is asked to leave a class by the DYT Director, due to behaviour or bullying issues, they will only receive a 50% refund.

Participants Release Statement
I desire that my child participate in DYT Performance Studio evening programming and activities, unless I advise the school director otherwise in writing. I agree that, having taken such precautions as in the studios discretion are deemed advisable, DYT and their staff will not be held respoisible for any sickness or accident to my child. If for any reason my child requires medical attention beyond that furnished by DYT, I agree to be responsible for any expense incurred. I covenant and agree to indemnify Do Your Thing Productions, its employees and staff and save them harmless from and with respect to al suits, actions and prosecutions by reason of any activity carried out by my child, whether on or off the Studio's property

Photo Release
I allow DYT Productions to take in class and performances video and photography of my child to be used for promotional purposes for The Actors Studio website and brochures ONLY. Performance CD's & DVD's will be provided for parents and students for only $5 per child.

I have read and fully support the Photo Release Agreement, and authorize photos to be taken of my child for the aforementioned purposes.
